- Intake forms — your library of intake forms and the rules. My intake forms holds the questionnaires (e.g. “Initial intake”, “Biweekly check-in”), and When to require is where you say who gets each one (new athletes, everyone, or people who bought through an integration) and whether it repeats.
- Feedbacks — the same thing, for your follow-up forms. See Intake form or feedback?.
- Answers — everything that has come in, with a button to see the intake form answers or the feedback ones.
The full path
1
You build the form
Create the questions however you like: text, choices, scale, slider, photos and more.
2
You say when it's required
A rule links the form to a trigger — a new athlete, all your athletes, or a purchase
through an integration (Kiwify, Hotmart, Sellyo).
3
The athlete answers in the app
They get a prompt and answer right in the app, one question per screen.
4
The right workout is delivered
If you set up delivery by answer, each answer unlocks the matching workout folder —
without you lifting a finger.
What changes for the athlete
When the intake form comes from a rule (required), the workout you delivered stays locked until they answer. That way nobody starts training before you know their history, injuries and goal. As soon as they submit, the workout is released the next morning at 6am (in their own timezone) — and the answers land in your dashboard.Intake forms include health questions (injuries, conditions, medications). That data is
sensitive — treat it with the same care you’d give a medical record.
