Each intake form is a questionnaire with its own name, and you can have as many as you want: one for beginners, another for the biweekly check-in, another for your fat loss crowd.

Create it

1

Open the forms hub

In the menu, go to Forms. Stay on the Intake forms tab to create an intake form, or go to Feedbacks to create a feedback — the steps are the same.
2

Click New intake form

Choose where to start:
  • Start from the basic form — comes with seven ready questions (goal, injuries, health conditions, medications, training experience, energy and photos). Just adjust.
  • Start from scratch — a blank page, you build everything.
3

Give it a name

In Form name, write something you’ll recognize later (e.g. “Initial intake”, “Biweekly check-in”). That’s the name the athlete sees in the app.
4

Add the questions

Use Add question and pick the type. Each question becomes a card you can edit, reorder (↑ ↓ arrows) or remove.
5

Check the preview

On the right you have the Preview — exactly what the athlete sees on their phone. It updates as you build.
6

Save

Click Save intake form. It goes into your library already Active.
On phones and smaller screens, use the Edit / Preview switch at the top to move between the builder and the preview.

What the list shows

On the tab of the type you opened, each row in the list shows:
  • Name — plus a note like “in 2 rules” if it’s used by a rule.
  • Questions — how many questions it has.
  • Answers — how many answers came in. Click the number to see them all.
  • Status — Active or Inactive.
The options menu (⋯) on each form gives you Edit, Activate/Deactivate, See answers, Export (.xlsx) and Delete.

Deactivate instead of deleting

Deactivating is reversible: the form stops being required and stops being sent, but the answers you already collected stay put.
Deleting a form also removes the rules that use it. If you just want a break, prefer Deactivate.
A short form gets answered; a long one gets abandoned. Start with 5 to 8 questions — you can ask the rest on the next check-in with a recurring form.
