When the athlete finishes the intake form, they see a completion screen. By default it’s just “Intake form submitted” — but you can turn that moment into your pitch, an offer or an invitation. It’s the best moment to talk to them: they just answered, they’re engaged, and the phone is in their hand.

Build the completion screen

1

Open the form

In Forms → Intake forms, open the form and scroll to Completion screen (optional).
2

Choose the media

Pick Image or Video.
  • Image — you upload the file.
  • Video — paste the embed code (the <iframe>) from YouTube, Vimeo or SmartPlayer. The video is shown in its original format, landscape or portrait.
3

Write the copy

Fill in Title and Subtitle (both optional).
4

Add the button

Tick Add action button and fill in the button text and the link (e.g. a coaching page, a group, a product).
5

Save

Click Save intake form. The preview on the right shows how it looks.

Controlling the pace

Two settings stop people from closing before they see it:
  • Hold the close (✕) — the close button only appears after X seconds.
  • Delay the button — the action button only appears after X seconds (useful when the video needs to be watched before the offer).
If your video already has its own button inside it, leave the screen without a button: the athlete closes with the ✕ and all the attention stays on the video.

Decline screen (downsell)

You can add a second screen shown when the athlete declines the offer. Turn on Decline screen (downsell) and build it the same way: media, title, subtitle and button. The “no thanks” text is yours too — you can change it to something like “Not now, thanks” or “Just the workout, please”. With no downsell configured, declining simply ends the form.

A different screen per answer

Want to show one video to people chasing fat loss and another to people chasing hypertrophy? Use Completion screen by answer (optional).
1

Turn the block on

Scroll to Completion screen by answer (optional) and click Enable.
2

Pick the key question

Select the question that decides the screen — it has to be single choice or cards. Multiple choice doesn’t work here, because the athlete could tick answers from two variants at once.
3

Build the variants

In each Variant, tick the answers it applies to and build the screen (media, copy, button, downsell).
If the athlete’s answer matches a variant, it replaces the default completion screen. If it doesn’t match any, they see the default one.
You need at least one choice question in the form to use this — the dashboard tells you when there isn’t one yet.
