When you click Add question in the builder, you pick the component type. Each one solves something different — here’s what they do and when to use them.

Questions

Free text

A text box for the athlete to write whatever they want. Use it for injuries, history, “what brought you here”.

Number

Numbers only. Good for age, weight, time available per session.

Single choice

A list of options where the athlete picks one. It’s the most used type — and one of the few that can drive workout delivery.

Cards (button choice)

Same as single choice, but the options show up as large buttons in the app. Looks better and is faster to tap when options are short (e.g. “Home”, “Gym”).

Multiple choice

The athlete can pick several options. Use it for “which days do you train”, “what equipment do you have”.

Linear scale

A rating ruler (e.g. 1 to 10) with labels at each end. You define:
  • Minimum and Maximum
  • Minimum label and Maximum label (e.g. “Low” and “High”)
  • Scale color — whether a high score is good (green at the top), a high score is bad (red at the top) or neutral (no judgement, just a blue ramp)
Perfect for energy, pain, sleep quality, stress.

Slider

A sliding bar. Besides minimum and maximum, you can set:
  • Unit (e.g. cm, kg, min, %)
  • Starting value — where the slider begins
Good for weight, height, available minutes — anything with a wide range.

Photos (front/side/back)

Asks for assessment photos. You choose which angles you want, out of five: Front, Back, Right side, Left side and Free pose. A new question starts with Front, Back and Right side ticked. Example photos (optional): you can upload up to 6 images showing the right pose. The athlete sees them above the question, before the upload slots.
Example photos are worth the trouble. They’re what turns a pile of random angles into an album you can actually compare over time.

Terms of use (acceptance)

An acceptance question, for the athlete to read and agree to before moving on.
  • You add up to 10 documents, each with a name (e.g. “Terms of use”) and a link. The athlete opens it in their browser.
  • The acceptance text is editable (the default is I have read and accept the documents above).
  • The required toggle becomes Acceptance required to continue.
Kampp keeps proof of the acceptance: date, IP, device and the exact links that were on screen. You see all of it in the athlete’s answer, and in the spreadsheet the column comes out as Accepted or Not accepted.
A terms question needs at least one document with a link before you can save it.

Screens that aren’t questions

Pause screen

A screen with just a title and some text — no field to fill in. Use it to give a breather mid-form (“Great! Now let’s talk about your training”) or to explain why you’re about to ask something sensitive. It accepts an icon (emoji).

Video

A screen with a video, which you can drop anywhere in the form. Paste the video embed (YouTube, Vimeo or SmartPlayer) and, if you want, a title and some text. The app respects the format, landscape or portrait. Use it to introduce yourself, explain your process, or set something up before a hard question. Up to 6 images the athlete swipes through. Use it to show your method, social proof, or to explain a choice visually before asking about it.

Settings that work on any question

Subtitle

Small text under the question title. Use it to instruct: “If yes, describe it. If not, write ‘None’.”

Helper text per option

On choice questions, each option can have an extra line of explanation. Great for telling similar options apart (“Mixed — I train at home and at the gym”).

Conditional text field

Tick Show a text field when certain answers are selected and choose which options reveal it. The classic case: they pick “Yes, I have an injury” and only then the “Which injury?” field appears. You set the label, the placeholder and whether the answer is required.

Required answer

Turns on the lock: without answering that question, the athlete can’t move forward.

Order

Use the ↑ ↓ arrows on the question card to change its position. The athlete answers in the order you build, one question per screen.
Only Single choice, Cards and Multiple choice can be used as conditions in delivery by answer. The key question of the completion screen accepts only Single choice and Cards. If you want to route workouts by goal, ask the goal with one of those types.
Carousel images and the photos the athlete uploads are stored securely — the photos show up in their profile, next to the answers.
