The intake form is answered inside the app, on the athlete’s phone. It’s worth knowing that screen: it’s what you’re building when you create the questions.

What they see

1

The invite

When they open the app, a prompt appears: “You have a new intake form” (with your name, when you have a coach profile set up). The button says Answer now.
2

One question per screen

Questions come one at a time, with a counter (“3 of 8”) and a Continue button. On the last one, the button becomes Submit.
3

The completion screen

Once done, they see “Intake form submitted” — or the completion screen you built, with your video and offer.

Details that matter

  • Required questions block them from moving forward until answered.
  • Photos can be taken on the spot or picked from the gallery.
  • The prompt never opens during a workout — Kampp waits for the session to end.
  • If they close it halfway, the invite comes back later.

And the workout?

If the form came from a rule (required), the workouts you delivered stay locked until they answer. After submitting, the workout is released the next morning at 6am in their timezone. If you set up delivery by answer, that’s the moment the right folder is unlocked for them.
Want to see how it turned out? Send the form to yourself (or a test account) and answer it in the app — it’s the fastest way to see the real thing, video and all.
Older intake records, from the previous format, are still visible in the athlete’s profile marked as a previous record. They don’t disappear — they just aren’t edited there anymore.
