# Which form to use?

> Three types: two questionnaires with different roles, and a request.

The **Forms** menu has three types. **Intake** and **feedback** are questionnaires: same
structure, same builder, what changes is the **role**. **Lab work** is something else —
it's a **request**, which is why it has no builder and no questions.

## The difference in one sentence

> **The intake form authorizes you to prescribe. The feedback keeps track. The lab
> request asks for a document.**

| | **Intake form** | **Feedback** | **Lab work** |
| - | - | - | - |
| What for | The full picture of who that person is | The short update on how it's going | The tests you need to see |
| When | At the start, and every so often | Often, usually every month | As part of routine follow-up |
| What it is | Long questionnaire: health, history, restrictions | Short questionnaire: three or four questions | A written list (and the lab form, if you have one) |
| They send back | Answers | Answers | The results, as a PDF or photo |
| Locks the workout? | **Yes**, if it comes from a rule | **Never** | **Never** |
| Priority | **Wins** over any questionnaire in the same month | Gives up its turn | Doesn't compete with anyone |

## Why feedback doesn't lock the workout

Because it doesn't need to. The intake form is what gives you the basis to prescribe
safely — without it, delivering a workout is guesswork. Feedback is follow-up: holding
back the workout of someone who pays on time because a monthly check-in went unanswered
would be out of proportion.

<Note>
  Even the intake form only locks the workout when it comes from a **rule**. Sent by hand,
  it's a request, not a requirement.
</Note>

## The priority rule

This is the point that saves you from the most common mistake when you use intakes and
feedback together.

Say you set the intake form to repeat **every 3 months** and the feedback **every month**.
In the third month, both land together.

**The athlete only gets the intake form.**

<Steps>
  <Step title="Month 1 and 2">
    They get the feedback, as usual.
  </Step>

  <Step title="Month 3">
    The intake form comes in. That month's feedback gives up its turn — the intake form
    already asks everything it would ask, and in more depth.
  </Step>

  <Step title="Month 4">
    The feedback is back, on its usual rhythm. Its cycle isn't pushed back or thrown off.
  </Step>
</Steps>

On the athlete's profile, that send shows as **"Not sent — the intake form of the month
took priority"**, so you don't think they ignored the form.

And the feedback cycle isn't pushed back: it returns next month, on its usual date.

<Tip>
  The window is the **month**, not the day. An intake form on the 1st and a feedback on the
  15th are in the same month, so the feedback gives up its turn — getting two
  questionnaires in the same fortnight is exactly the pile-up this rule exists to prevent.

  It's a calendar month: an intake form on 28 March and a feedback on 2 April are different
  months, and both go out.
</Tip>

<Note>
  **Lab work stays out of this.** It isn't a questionnaire: asking for blood tests doesn't
  compete with asking how the month went, so an intake and a lab request in the same month
  both go out.
</Note>

## Choosing the type

The type is set **when you create it** and doesn't change afterwards: the tab you're on
decides. On the **Intake forms** tab the button creates an intake form; on **Feedbacks**,
a feedback; on **Lab work**, a request.

<Warning>
  You can't turn an intake form into a feedback later. The type decides which one locks the
  workout and which one wins the month — changing that midway, on a form that is already used
  by rules and holds answers, would change the behavior without anyone noticing. If you got
  it wrong, duplicate it and rebuild it under the right type.
</Warning>

## Where each one shows up

The three types stay **separate everywhere**:

* In the **Forms** menu, each on its own tab.
* On the athlete's profile, in their own sub-tabs inside **Assessments**.
* Under **Answers**, with a button to switch between them.

<Note>
  Want to build your first one? See [Create an intake form](/en/create-intake) — the steps
  are the same for intakes and feedback. For lab work, which skips the builder entirely,
  see [Requesting lab work](/en/request-lab-work).
